Wrestler Release and Waiver Form
Basics Plus Wrestling Camps requires that the parents or guardians of all campers sign our Release and Waiver form.
Campers will not be accepted unless this form is signed by a parent or the camper's Legal Guardian.
Physician Approval and Physical Form
All wrestlers must provide Basics Plus Wrestling Camps with their health histories and obtain physician approval to attend
one of our camps.
Travel Itinerary Form
Mandatory form for all wrestlers traveling by plane, train, or bus and requiring pick-up.
